How to Claim

1. Take a treatment in opd at any MBBS or higher consultant.
2. Collect all bills of consultation charges & prescribed medicines bill.
3. Post bills at Claimdesk or courier us within 2 weeks after last doctor visit or treatment completion.
4. Get direct cash amount credit into your bank account after approval of 1 week.

Please Note

• The doctor fee will be actual amount subject to maximum of ₹ 500 per visit.
• Medicine Bills If your prescribed medicines bill is INR 2500 or above, you will receive the maximum money back INR 2500.
• Maximum money back request unlimited times can be placed to get it as on actual consultation charges & prescribed medicine bills subjected to the said limit  INR2500.
• All moneyback request are applicable for active membership card holder only.
• Not applicable for Dental & fancy cosmetic treatments, hospitalization, No Coverage on Baba tantrik, Homeopathic, “Jhola chhap doctors” or similar in nature.
• This plan is not applicable for minors.
